Gist of Medi-claim Scheme for Ex Employees of the Bank & their spouse under Staff Welfare Scheme

A medical cover to all eligible Ex-Employees and their spouses, under which they shall be covered under a Floater Group Mediclaim policy of Rs 2 lac per family. In case of deceased employee their spouses shall also be covered under this policy. In case of employees who have opted / opting for VRS or resigning, only those employees having completed either 30 years of service or attained 55 years of age at the time of joining the scheme ,shall be eligible to become a member. All retired eligible employees/ officers/ their spouses in order to be covered under this policy shall have to become member of this scheme by submitting their duly filled application alongwith a yearly membership fee of Rs 200/-

Scheme for Providing Identity Cards to Retired Employees Under Staff Welfare Scheme

Bank has decided to issue Identity Cards to all its Ex-Employees for the purpose of establishing identity of the holder as an Ex-Employee of the Bank. In order to have correct particulars / latest photograph on such cards we are enclosing herewith a proforma, wherein you may obtain the details of all pensioners / account holders ex-employees and send the same to us at the earliest to enable us to issue the identity cards to them. We are pleased to inform you that as a gesture of Goodwill, such cards shall be issued free of cost initially. However if an ex-employee requests for issue of duplicate card in lieu of lost / damaged card, request letter alongwith a fresh proforma accompanied with a DD/MC of Rs 50/- [ Rupees fifty only], drawn in favour of Punjab & Sind Bank Employee’s Welfare Fund Trust be forwarded to HO PF/Pension Deptt. In case of lost card request must be accompanied by a copy of FIR while in case of damaged card, the same shall have to be sent alongwith such request

We are sure that the introduction of the Identity Card to all our retired employees will act as an means of belongingness with this esteemed institution by them. Please ensure that all such employees are intimated about this scheme , so that they can be issued such cards at the earliest. Also note to put up a copy of this circular on the Bank’s Notice Board. We are also placing this circular on bank’s intranet as well as Internet site, for their convenience.
